To establish membership with FourLeaf, you will need:
Government Issued ID
& Social Security Number
An unexpired ID can include Driver’s License, State-Issued Identification, and US Passport Book.

Proof of Current Address
If your ID shows a previous address or a PO Box, acceptable items are:
- Current utility bill
- Recent official payroll check stub
- Most recent tax return
- Property tax bill
- Current mortgage statement
- Official letter from a US government agency
Joint Owner
For joint accounts, you’ll need the same information for both applicants. Each joint owner must complete their part of the application.
Mobile Device
For online account opening you must also have your mobile phone available for verification. Also have handy your username and password for the financial institution you plan to fund your new account with.
